Terms of Service

1. Agreement

By scheduling, requesting, or receiving services from Dependable First Solutions, you agree to these Terms. If you do not agree, please do not use our services.

2. Services Provided

We provide drug and alcohol testing, specimen collection, and related compliance services for employers, individuals, courts, and other authorized parties. Services include but are not limited to pre-employment screens, random testing, post-accident testing, court-ordered screens, DOT and non-DOT testing, rapid testing, and mobile / on-site collections.

3. Appointments & Cancellations

We aim to accommodate requested appointment times whenever possible. Appointments may be rescheduled by contacting us during business hours. Missed appointments without notice may be subject to a fee.

4. Identification & Documentation

Donors must present a valid, government-issued photo ID. For court-ordered or DOT testing, supporting documentation may be required. Failure to provide required information may result in the test being declined or invalidated.

5. Test Results

Rapid screen results are provided at the time of collection where applicable. Lab-confirmed results are delivered through secure channels to the authorized party. We are not responsible for delays caused by third-party laboratories, couriers, or Medical Review Officers.

Test results are reported as collected and interpreted under applicable standards. We do not provide legal, medical, or employment advice based on results.

6. Payment

Payment is due at the time of service unless other arrangements have been made for established business accounts. We accept major credit cards, debit cards, and approved corporate billing.

7. Confidentiality & Compliance

We maintain strict chain-of-custody and confidentiality procedures consistent with CLIA, DOT 49 CFR Part 40, and HIPAA where applicable. See our Privacy Policy for more information.

8. Limitation of Liability

Dependable First Solutions' liability is limited to the cost of the service provided. We are not liable for indirect, consequential, or incidental damages arising from the use of our services, except where prohibited by law.

9. Changes to These Terms

We may update these Terms periodically. Continued use of our services after changes constitutes acceptance of the updated Terms.
